Our Philosophy
We are dedicated to the inner strength of families and the preciousness of each child. We feel the role of the pediatrician and the pediatric nurse practitioner is to provide the highest quality health care, to assist parents in the task of encouraging their child's development and to be advocates for the health and safety of all children from newborn to young adult.

testing at the Westlake office are available:
Monday-Thursday: 9-11:30am, 1-6:30pm
Friday: 9-11:30 am, 1-3:30pm
Saturday: 9:30-11:30 am

We ask that you
arrive 10 minutes early for your
appointment to expedite the check-in
process. This will help us to keep
our schedules on time. We will be requesting a copy
of your drivers license and insurance card
at every visit. Please bring these
documents to every visit to avoid delays.
A charge of $50.00 will be
assessed to your account if you "no show " for an
appointment. This is not covered by
insurance.
Minors under 18 years of age must be accompanied by a parent or
legal representative to every visit. Consent forms to treat a
minor in the absence of the parent or legal guardian can be found on the
insurance link. Written documentation is required before a minor
can be seen.
IF YOU DO NOT HAVE YOUR CURRENT INSURANCE
CARD WITH YOU AT THE TIME OF YOUR VISIT, YOU WILL BE REQUIRED TO PAY
IN FULL OR RESCHEDULE FOR A TIME WHEN YOU
HAVE YOUR INSURANCE CARD WITH YOU.
If your child is 18 years of age or older,
an authorization to release medical information must be on file in the
chart or we cannot discuss medical information over the phone with the
parent. These forms are available on the insurance link.

 Having
a Baby?
Prenatal tours are offered monthly at the Westlake office. Meet one of the
doctors, tour the office and learn valuable information about our
practice.
Call 440-871-5100 to schedule a tour.
Requests for medication refills will be phoned to your
pharmacy or will be ready for pickup within 24 hours of the request. A nurse
will contact you to confirm the medication and dosage. Medication cannot
be refilled without this information.
Refills for long term medications require a follow up
every three months and a yearly physical. This includes medications for
asthma and ADD/ADHD.
If you use a mail order pharmacy, we do NOT fax refill
requests or new prescription orders. You need to mail any prescription,
so plan accordingly to receive your medicine on time.

FORMS
Forms for school, sports, camp etc. requiring an MD signature, can only be filled out if your child has had a physical within the last
year. Please allow 5-7 days if you are dropping off a form to be filled
out.
